As of April 1st, CMS will require updated paper 1500 and UB04 Health Insurance Claim forms. The 1500 Claim form will be updated to accommodate for the additional ICD10 characters that will be required on the form for the October 1st ICD10 Code implementations. The UB04 Claim form will include a check box to indicate that an ICD9 code has been included on the print form.
If your agency is currently utilizing the paper 1500 form or the paper UB04 form, we ask that you please contact the Customer Support Team (269-963-1040, option 1) as soon as possible to ensure your agency is updated with the appropriate forms. The Support Team will also conduct print testing with each agency to ensure compliance on April 1st.
