The methods of entering and tracking Care Teams for a patient has been updated in the new GLMI Demographics system. This section will still list the patient’s Providers and Physicians, but now has new features including history tracking and an automatic, integrated PECOS check for each physician added. In this post, we will take a look at the new Care Team section of Demographics and highlight a few of the new features.
Care Team Lists
The Care Team section consist of two lists. One displaying the Providers for the patient (left) and one displaying the Physicians for the patient (right). Both lists will default to display the active care team members, but both allow viewing historic information using the ‘H’ history toggles. The providers in the list are grouped by their selected type (Case Coordinator, Primary RN and Team Member) and the physicians are also grouped under their selected type (Attending, Consulting, and Other).
Q: How can I quickly see a physician’s contact information?

A: There are two ways to quickly view this information. (1.) Click the (+) next to the physician’s name to reveal their contact information. (2.) Contact information is displayed for the Attending Physician directly on the Care Team Tile of the Demographic Overview. In both of these areas, the information is pulled directly from the Physician Support Files.
Q: How can I remove a member from the Care Team?
A: Discontinuing a member from a patient’s Care Team has never been easier! Simply select the person in the list (Provider or Physician) and click the corresponding ‘Discontinue’ button. The person is immediately discontinued from the team and drops into the Provider or Physician history list for future reference.
Q: Interesting, how do I view this history list?
A: Selecting the ‘H’ History toggle activates the history view for each list. With this view active, the list is grouped into ‘Current’ and ‘Historical’ groups so you can easily see who is currently on the team and who was on the team. Note that the ‘H’ History toggle is only active when history is available for a certain list.
Adding Team Members
The Provider and Physician Add Overlays both function in very similar ways. For each, you choose a person from the corresponding support file, then assign a type. For providers there can only be one active Case Coordinator and Primary RN, while there can be as many Team Members as necessary. For physicians, there can only be one active Attending, Consulting and Other physician. Attempting to add another provider or physician to the restricted type will trigger a prompt asking if you would like the replace the current with the new selection. You may have also noticed that both add overlays feature the Save & Add Another. Along with all other overlays in the new GLMI Demographics, this feature is designed to accelerate the process of creating your care team.
Automatic Physician PECOS check
One very important area where adding a Physician differs from adding a Provider, is the integrated PECOS check. For each physician added to a care team, the GLMI Demographics system runs an automatic PECOS check. This check utilizes a PECOS physician checklist that is hosted in the GLMI Cloud. As you select each physician, you will see a PECOS indicator, displayed in the right side of the physician search box, displaying one of the following options:
Passed: The selected physician was found and their details match the list exactly.
Warning: The selected physician was found but their details do not currently match what was found in the list.
Error: The selected physician was not found in the PECOS checklist.
This great feature is available for both GLMI Cloud AND File Transfer systems, all that is required is an active connection to the internet.
